Well, even though I'm not going to be a Medic, I can see there being SubGuilds in the other Professions, so I guess I can still have an opinion on the matter. So long as it's not forced, I'd be all for it (though, since you said forced, I went with number 2.. heh).
If you can still be a well-rounded Medic that dabbles in everything, but has no bonus to anything, I'd be fine with it. Think General Physicians. But then, once you decide you really want to focus in an area, that should be your choice, not someone else.
